What insurance plans are accepted?

We accept most major insurance plans, including Aetna, United Healthcare, Molina, Humana, and others. If you don’t see your specific carrier listed here, there is still a high likelihood that we accept your coverage. In addition to the insurance plans listed above, we also submit claims via private pay, workers' comp, and automobile insurance plans.


What if I don't understand or disagree with the charges on my statement? 

Please call our office at 262-377-2900. We will work with you to answer your questions. If we cannot resolve the issue, you may need to contact your insurance carrier.
